Medical Cremation Form C
Form C is the Medical Certificate that forms part of the cremation application. It's completed by the deceased's doctor, who confirms they knew the person's medical history, saw them in the weeks before death, and examined them afterwards. The doctor must also be fully registered with the Medical Register of Ireland.
On the form, the doctor records the cause of death, confirms whether a post-mortem took place, and notes whether there's any reason to suspect the death wasn't from natural causes - if so, it must first be reported to the Coroner. They'll also confirm there's no financial interest in the death, and check whether the deceased had any implanted devices, like a pacemaker, that need to be removed beforehand to avoid damaging the cremator.
It's a routine but important step that helps ensure everything is in order before cremation can go ahead, giving families reassurance that this stage has been handled properly and with care.
